Patient Rights & Responsibilities
The patient or the responsible relative of the patient should provide correct and accurate all information about patient's present condition, illness, hospitalization, medication, hereditary conditions and any other condition that pertain to patient's health.
Patient must follow all rules and regulations of Hospital.
Precise timing of hospital visit should be followed.
Poor patients, below poverty line, beneficiaries of medical insurances /concession, patient included in the Municipal Medical Assistance schemes must submit all related documents to avail the scheme at the time of admission within 24 Hrs. of admission.
It is mandatory to have a responsible attendant 24 Hrs. by the patient.
It is necessary to have female attendant with female patient.
Only 2 persons can visit the patient at one time in visiting hours.
Patient should take care of their valuables and belongings. Hospital Management will not be responsible for any loss.
Below 12 Yrs. children are not allowed to visit the patient.
It is strictly prohibited for spitting, drinking, smoking, tobbaco and ghutaka chewing etc. If it is observed it is considered as an offence and strict action will be taken.
Respect Doctors, interns, nurses, ward boys and workers of hospitals. If any complaint is their; please contact hospital administration officer.
Avoid making any kind of noise like shouting, listening loud music, chatting etc. Maintain silence in hospital premises.
It is mandatory to treat other patient and their relatives in hospital with respect.
Follow the medication and instructions prescribed by doctor.
Avoid providing any medication without consulting a doctor.
Patients are not allowed to leave the hospital premises without prior permission of the treating doctors.
It is mandatory to clear all the bills of hospital in time.
Maintain cleanliness and hygiene in hospital premises. Use dustbin for garbage.
Avoid consuming food products near patient's bed, ward, corridor, staircase, patient waiting room. Use canteen on the ground floor for these purpose.
Avoid washing and drying of clothes and Tiffin in common toilets and washrooms of hospital.
It is strictly prohibited to misbehave as threat to a hospital building.
Return case paper before leaving hospital campus.
For case papers new / old; it is mandatory to present anyone of this -Aadhar Card /Pan Card / I Card.
The hospital is run by Charitable Trust so the bill amount is already minimal, so avoid request / argument for concession.
Tahasildar Certificate or Reshan Card (Yellow / Orange) is mandatory for income proof.
